Applications

Applications are processed as soon as they are received. Once your application is received, you will be notified as to whether or not we have a spot available or if you will be put on our wait-list. Applicants can remain on our wait-list for up to one year before having to reapply.

Instructions for applying are as follows:

1. Download the applicable application form.
2. Complete the form in its entirety. The applications are fillable pdf’s so you can just type in your responses.
3. Print off the form and sign where indicated.
4. Get a Commissioner of Oaths to sign the application form where indicated.
5. Attach a copy of your latest Notice of Assessment.
6. Either mail the form or email to the appropriate Regional Site Manager.
